Tia Dalma??
 

So I finished all the voodoo quests and sailed all the way back to visit Tia Dalma on Cuba. I went to the right while facing the island. Through the water and to the wooden walkway. All I found were plants that kicked my butt. Did I not go to the right place? Where is she??

There are 2 ways into that swamp. If you take the entryance that is far right you should see her when you come into the swamp feeding a gator.
