Anyone who thinks that the people who give out the suspensions/bannings and the people who develop the content are one and the same, you are dead wrong. Totally different group. Software developers would never be allowed to just sit and review chat logs.

The way that works is:
The filters are set to look for particular words, phrases, or context. The algorithms will flag and immediately send a response for outright swearing. This is ongame and immediate. It will also send a report to your account.
Other phrases and words do not send an immediate response to you, but send a notice to the reviewers that there was something questionable. They then go and pull the chat logs for review. (Here's where individual personalities come into play.)
I also believe that there are times when a reviewer will just sit and watch the chat scroll by at random. In-game live review of the chat. I can't prove this, but strongly suspect it.
And, I agree with Zep, I believe that they will target a person, or group of people, after handing out a suspension. They seem to come in bunches. Almost immediately after someone gets suspended, their friends, guildmates, and even others who just happened to be nearby start getting watched.
