I take it back. It does the same thing for me, too.
I get a 2-hit combo from the hack/slash, then a 3-hit combo from the flourish.
I did the same thing with dagger - skipped gouge and went straight to eviscerate. It does the same thing. The combo starts over again at eviscerate. I get a 3-hit combo from cut/swipe, and another 3-hit combo from eviscerate.
I think this is an actual bug.
