To preform the hand sign you have to hit Ctrl+Alt+HP+Pageup+Del when your character is emoting /HPhandsign!

As you can see Ben was preoccupied with hitting all those buttons and typing the needed emote and wasn't able to also hit the left mouse click button to fire the cannon.
