While it's best NOT to randomly friend or accept random friend and crew requests - there is no real downside to taking a moment to heal or aid a fellow pirate in your travels. After all, you never know when Karma dictates that you need a hand back.

*NOTE: There is a difference between 'aiding' someone and 'kill stealing' from them. Learn the difference. I tend to not attack someone else's opponent; I choose to heal them or cast Curse or Shackles on their enemy to lend a hand.

It can never hurt to have too many friends
